RULES, TERMS, & CONDITIONS
All students will be charged an annual membership fee.
Membership fees will be automatically charged to your account on the month of your anniversary date each year that your student is active.
We encourage you to make-up all classes missed due to illness, holiday closing, schools closing due to bad weather, etc. However, make-up classes must be within 30 days of the missed class. You must schedule(online or call) your make-up class ahead of time so that we do not overbook another class.
I understand that enrolments are YEAR ROUND and I must LOGIN TO MY ACCOUNT AND REQUEST TO DROP ENROLLMENT in order to discontinue classes at any time. If I fail to do so BEFORE THE 1st I will be financially responsible for the month’s tuition.
Please note: You are responsible for payment for your student’s classes WHETHER OR NOT YOUR STUDENT ATTENDS CLASS.
VIP ANNUAL PRESENTATION:
Each student will showcase the skills he/she has achieved in his/her weekly classes. Each student will receive a medal in accordance with the skill level he/she has accomplished. By enrolling in classes at PowerHouse Tumble and Cheer your student will be automatically enrolled in the VIP performance. VIP will be automatically charged to every active student’s account along with the monthly charges. If you wish to refuse participation you must submit an online drop request from the VIP before the 1st of the next month.
WHAT TO WEAR:
Boys or Girls may wear T-shirts and shorts OR Leotards for Girls. NO chewing gum or dangling jewelry. Hair should be pulled neatly and securely away from the face so that is stays up for the entire workout. Girls should not wear large hair ornaments that may cause discomfort during activity. All students should have activity-appropriate footwear during class. Personal items should be left on the designated student shelves. Jewelry should not be worn during classes. PLEASE LEAVE JEWELRY ARTICLES AT HOME. This facility's staff will not be responsible for ANY items that may be lost or stolen. Be sure your student's personal items are marked with their name.
ARRIVAL AND PICKUP:
Be sure your student arrives 5 minutes before his/her scheduled class time. Please pick up your student on time. Please inform us if you know you will be late picking up your student. Instruct your student to wait inside the building and you should escort them from the building to your car. During peak times the street is crowded. Please take into consideration that our students may include young children. Please drive slowly and carefully. Do not take a chance on your student running to and from your car.
I understand by the listed student(s) participating in class and/or other activities at PowerHouse Tumble and Cheer, that the instructors and/or the owner are not responsible or liable for personal injury and/or accidents which may occur as a result of participating in this program or by being on this property. I, hereby, give my consent for the instructor, after reasonable attempts to contact me have been unsuccessful, to seek medical help or initiate first aid for the listed student(s), if necessary.
I hereby consent to and authorize the use and reproduction by POWERHOUSE TUMBLE AND CHEER OF WEST MONROE LLC. or anyone authorized by POWERHOUSE TUMBLE AND CHEER OF WEST MONROE LLC. of any and all photographs that have been taken of me and/or the listed student(s) this day for any purpose, without compensation to me. All negatives and positives, together with the prints, are owned by POWERHOUSE TUMBLE AND CHEER OF WEST MONROE LLC. and the authorized photographer. POWERHOUSE TUMBLE AND CHEER OF WEST MONROE LLC. and the authorized photographer reserves the right to use these photographs in any of its print or electronic publications. I understand that POWERHOUSE TUMBLE AND CHEER OF WEST MONROE LLC. has the right to use any photographs, videotapes or any other form of media for publicity, advertising for any legitimate purpose without limitation or compensation.
**All ACCOUNTS ARE REQUIRED TO SET UP AUTO PAY IN ORDER TO ENROLL IN CLASSES.ANY CARD SAVED ON YOUR ACCOUNT WILL AUTOMATICALLY BE DEBITED FOR YOUR FULL ACCOUNT BALANCE THE FIRST BUSINESS DAY OF THE MONTH.
UNPAID BALANCES OVER 30 DAYS PAST DUE WILL BE SENT TO A PROFESSIONAL COLLECTIONS AGENCY.
I understand that tuition does not change regardless of a 2,3,4 or 5 week month. Insufficient checks will have a charge of $15. After the second insufficient check it will be necessary to pay in cash from that time forward. Payments that are 15 days late will have a $5 late charge and an additional $5 charge will be added for each additional 30 days late.

If you have signed up for auto pay your card on file will be charged for the full balance on your account. You may sign into your account to create a payment on a different card before the end of the month if necessary.
*To sign up for Autopay you must save your card information on your account Payments Page.
*Visa Prepaid cards are accepted through autopay. Please be advised there must be sufficient funds to cover the entire balance being charged for autopay to work. If your card is declined additional fees may apply.
I hereby authorize (if online payment is made or autopay information is provided) this facility to charge my ACH draft, or credit card account.
I represent and warrant that if I am purchasing something or paying for a service from this facility or from other merchants through this facility that; any credit card or auto pay information I supply is true and complete; charges incurred by me will be honored by my credit card company or financial institution; I will pay the charges incurred by me at the posted prices, including any applicable taxes, fees, and penalties.
It is always the responsibility of users of the Auto pay service to contact PowerHouse Tumble and Cheer directly if they do not receive their billing statements either electronically, by mail, or otherwise, and if any electronic Bill Payment transaction initiated by such user fails or is not processed in a timely and accurate manner.
It is the responsibility of each user of the Auto pay service to review bills for electric services in a timely manner and to bring any errors, omissions, or questions to PowerHouse Tumble and Cheer's attention.
Suspected errors or omissions in an electric bill or questions about the Auto pay service or any failed electronic Bill Payment should be promptly directed to PowerHouse Tumble and Cheer's office staff.
I have read and agree to abide by all Rules, Terms and Conditions of PowerHouse Tumble and Cheer including but not limited to “WAIVER”, the “TUITION POLICY”, the “DISCONTINUATION POLICY” and the “MAKE-UP POLICY” in order for my student to be enrolled at PowerHouse Tumble and Cheer.
